Sub-Acute Rehabilitation
At Lexington Health Care we offer sub acute rehabilitation and medically complex nursing services for patients who require skilled nursing care after a hospital stay, surgery or short spell of illness. Within the past year, Lexington has successfully rehabilitated over four thousand patients offering physical, occupational and speech therapies up to 7 days a week. Depending on the diagnosis, Lexington also offers specialized sub-acute programming which includes orthopedic, pulmonary, cardiac, wound, stroke and movement disorders. Our high success rate in clinical outcomes makes a case for the great care we offer.

It is important to us that our patients receive the best care possible. While you are visiting with us, we want you to enjoy the recent remodeling of our sub-acute area—a reflection of our commitment to you. We have included many amenities to make your stay more enjoyable, comfortable and relaxing. This area features completely renovated patient rooms containing flat screen TVs with pillow speakers, wireless internet and electric beds.

Memory Care
Our Memory Care program is designed for both men and women. Innovative programming with an emphasis on holistic care is the cornerstone of our residents' care. We always strive to provide the best possible training for our staff and seek to create a culture of "working where our residents live" as opposed to "our residents living where we work." At Streamwood, Wheeling and Orland Park, we also offer a Memoirs Program, an intimate environment where we focus solely on the needs of women who have mild- to mid-stage memory loss. A newly renovated living space and specially trained staff help to create a warm, welcoming new place to call home. The Memoirs Program is based upon intentionally creating purpose based activities each day. This helps enable residents maintain both independence and high cognitive functioning for as long as possible.
