At Lexington Square, our rehabilitation gyms are the perfect place for your Medicare Part B physical therapy and rehabilitation workouts. We offer rehabilitation services to strengthen and gain momentum on your wellness journey.

Lexington Square is also part of a comprehensive network of services including Skilled Nursing Care (link for individuals requiring physical, occupational or speech therapy after an acute illness, injury or hospital stay.

Skilled nursing and post-acute services coordinate the efforts of doctors, nurses and therapists to treat patients with a history of orthopedic, pulmonary, cardiac, wound, stroke and movement disorders in specially designed facilities to transition you back home after an illness or injury.

Our network offers ten health care centers based throughout the Chicagoland area, including care centers conveniently located on our Elmhurst and Lombard independent living campuses.

Life Care Contract Option

You have the option to include a Life Care Contract as a resident of Lexington Squares, which entitles you to preferential choice to our skilled nursing centers in the Lexington Health Network if and when needed.

From the moment you arrive, we discuss your treatment and recovery goals in collaboration with your physician. Your care team will create an individualized plan tailored to your needs, and we will communicate with you every step of the way as you progress toward those goals. Many of our patients come from the hospital to strengthen themselves after a surgery or illness.
